Evaluation of 1,000 Days Together

This is the first narrative report for The Together Program (TTP) evaluation being undertaken by the Home2Health team at The University of Notre Dame Australia. Prior to describing progress on the evaluation to date, the TTP aims, scope, and model of care are summarised as a contextual backdrop to the evaluation. TTP commenced as a three-year pilot early November 2023 and is funded by the Sisters of St John of God. The evaluation is funded by the Minderoo Foundation. This report covers the first 12 months of operation.
