Hospital Responses to Homelessness

Royal Perth Hospital

The Royal Perth Hospital Homeless Team is a collaboration between Royal Perth Hospital and the Homeless Healthcare General Practice designed to support patients who are experiencing homelessness. The RPH Homeless Team helps to link patients with General Practitioners and coordinates their care so they can receive ongoing support in the community. The RPH Homeless Team works to link patients with support services that will help to address their underlying psychosocial needs. The Home2Health Team is conducting an ongoing evaluation of the impact of the Royal Perth Hospital Homeless Team.  

St Vincent’s Hospital Melbourne

Throughout 2016 and 2017, the Home2Health team were involved in the evaluation of a suite of four homelessness services operated by St Vincent’s Hospital Melbourne (SVHM): ALERT, The Cottage, CHOPS and Prague House.
